Thank you for your interest in enrolling your child in our educational program!
Below you will find the information regarding completing the application to be part of our Early Head Start/ Head Start waiting list.
Please complete the application for our Program, Queensbridge Family Child Care, at the link below:
Under the PREFERENCE section choose the program option from below:
If your child is 6 weeks to 2 years and 11 months the Program is: EHS Direct 2024-2025
OR
If your child is 3 years to 5 years old the Program is: Headstart 2024-2025
Select Queensbridge FCC as the Program.
Once selected from our Waiting List you will need to provide further documentation that is necessary for the completion of this application.
(Please see the list of documentation needed below)
You must submit the following:
1. Picture ID (of parents)
2. Proof of Address
3. Birth Certificates (submit for all children even those that do not need child care)
4. Proof of Income (pay stub (for a month)/w2 form/Taxes/Notarized Statement-if paid in cash or if you are unemployed)/Food Stamps letter
5. Child's Health Insurance Card
6. Immunization Record (Yellow Card)
